Job summary

Qualifications

• High school diploma or equivalent is required.
• Minnesota Community Health Worker Certificate from a Minnesota State-approved curriculum.
• Knowledge of Native American populations and culturally responsive health practices.
• Strong interpersonal and communication skills.
• Ability to provide patient education, care coordination, and health navigation.
• Ability to work independently and collaboratively.

Responsibilities

• Provide health education and coaching to individuals and groups.
• Coordinate care and navigate health systems for clients.
• Assist with accessing healthcare services, referrals, and benefits enrollment.
• Facilitate communication between clients, providers, and community resources.
• Document client encounters and Medicaid-related records in the clinic's EHR.
