Hennepin Healthcare is hiring a Community Health Worker for the Women’s Health Clinic in Minneapolis. The role focuses on helping patients navigate the healthcare system, connect to community resources, and promote self-management and health promotion activities.

Responsibilities include care coordination, documenting progress, and supporting patients with appointments and transportation. Qualified candidates have MN CHW certification and strong communication skills in English.

Qualifications

• Minimum qualifications include CHW certificate completed within 18 months of hire.
• UMPI will be acquired during onboarding.
• Driver's license and proof of insurance if field visits are required.
• MN Health/Home care care coordination certifications may apply.

Responsibilities

• Provides care coordination and navigation to connect patients with community resources.
• Collaborates with patients, families and care team to address needs and care plans.
• Encourages patients to take steps toward health promotion and chronic disease management.
• Performs needs assessments and health screenings for patient populations.
• Documents care coordination and education in electronic health records.
• Facilitates transitions of care and coordinates services across continuum.
