Job summary

Good Shepherd Health Care System is seeking a Community Health Coordinator to support the Community Health & Outreach programs. You will handle customer service, referrals, scheduling, data entry, resource coordination and event support within the Community Health Education department.

Requirements include a high school diploma and customer service experience, with preferred 3 years in an admin/office setting and event planning. Bilingual English/Spanish is a plus.

Responsibilities

• Provides customer service for all Community Health & Outreach departments (phone, walk-ins, appointments).
• Assigns new referrals to appropriate department or Community Health Worker.
• Maintains an up-to-date list of community resources and services.
• Schedules and completes data entry for CPR/BLS classes.
• Manages data collection and inventory for community benefit services.
• Oversees department inventory and CareVan scheduling/appointments.
• Assists with scheduling and coordinating community classes and events.
• Gathers materials for 40+ events per year and coordinates staffing.
• Supports Employee Wellness program and data entry.
• Assists with health messaging and social media postings.

Schedule: Mon - Fri/First Shift/40hours

Definition Of Position

Provides coordination and support for Community Health & Outreach programs and departments.

Responsibilities

Essential Job Functions:

• Provides customer service for all Community Health & Outreach departments (phone calls, patient/community walk-ins, and appointments for ConneXions, Medicare counseling, community classes, and more.
• Assigns all new Community Health and ConneXions referrals to the appropriate department and/or Community Health Worker.
• Maintains a current, up-to-date list of all community resources and services.
• Schedules, coordinates, and completes data entry and collection for all CPR/BLS classes.
• Data collection and inventory management for all community benefit services.
• Manages all department inventory and warehouse items.
• Provides direct support for CareVan scheduling/appointment management.
• Assists with scheduling and coordinating all community classes and events.
• Gathers and organizes all necessary materials for 40+ events/year.
• Coordinates staffing for each community event.
• Provides support for Employee Wellness program, software, and data entry.
• Assists with community health messaging/advertising in the form of bulletin boards, brochure management, event/class/health messaging social media postings, and more.
• Assists with data entry, document creation, filing, tracking, reporting.
• Maintains up-to-date records for all CHO programs, events, classes, and grants.
• Assists with other departmental projects, as needed.
• Oversee the day-to-day operation of Community Health & Outreach programs and departments by aiding as needed.
• Communicate effectively and therapeutically with people from every socioeconomic background.
• A well-organized self-starter who works well independently, as well as a team member.
• The employee supports the hospital mission, vision, values, policies, and procedures.
• Participates in required education for DNV programs as applicable to position. (reference program education curriculum).
• Performs other related duties as assigned.

Qualifications

Education

Required: High school diploma or equivalent.

Preferred: Supplemental courses in office work preferred.

Experience

Required: Customer service experience.

Preferred: Prefer 3 years’ previous experience working in an administrative/office setting. Previous event planning/management experience.

Other

• Excellent customer service skills
• Multiple phone line telephone system with excellent telephone etiquette skills
• Excellent skills in English, punctuation, spelling, grammar, proofreading, attention to detail
• Proficient in computer skills, typing, Microsoft programs (Word, Excel, Publisher), telephone systems, and copy/fax machines.
• Bilingual English/Spanish preferred.
