Job summary

The PATH Community Health Worker (PCHW) at Connecticut Pediatrics @ CHC bridges individuals with substance use disorders to healthcare and social services. They provide outreach, recovery support, care coordination, and system navigation, advocating for clients while implementing the PATH outreach plan.

Responsibilities include street outreach and shelter visits, partnerships with community organizations, data entry for PATH within 7 days, and education on harm reduction.

Qualifications

• High school diploma or GED with 2+ years of relevant work experience.
• Lived experience of mental health or substance use recovery.
• Understanding of local SUD challenges and community culture.
• Training in motivational interviewing and crisis intervention.
• CHC-approved CHW training must be obtained within 90 days of hire.

Responsibilities

• Build trust with individuals using substances and connect them to harm reduction and treatment.
• Engage in outreach in shelters, community organizations, and other settings to empower clients.
• Advocate for clients and facilitate timely referrals and access to care.
• Collaborate with care teams to ensure holistic, continuous support.
• Collect and submit PATH Program data within 7 days of encounters.
• Provide community education on harm reduction and disease transmission.
• Assist with housing, transportation, and insurance enrollment as needed.
