Job summary

PermiaCare seeks a Community Health Worker to aid individuals with substance use disorders in navigating SUD/mental health and medical services in Texas. The role requires obtaining State CHW certification within 6 months of employment and maintaining confidentiality under HIPAA.

The position involves office and field settings, with a 40-hour work week and potential for flexible hours. A valid Texas driver's license and background checks are required.

Qualifications

• High School Diploma or GED required.
• At least 2 years in substance abuse and/or mental health.
• Must obtain state CHW certification within 6 months of hire.
• Must have valid Texas driver’s license and pass background checks.

Responsibilities

• Provide direct support services to clients as directed by supervisor, including navigating PermiaCare and systems of care.
• Provide indirect support services to clients as directed by supervisor.
• Complete required documentation within 24 hours of service delivery.
• Educate community members on health and wellness resources.
• Connect individuals with local healthcare providers and social services.
• Maintain confidentiality of sensitive records in compliance with HIPAA.
