Cleveland Clinic Health System offers a Community Health Worker Apprentice program based in Cleveland. The 25-week apprenticeship provides hands-on training under a CHW preceptor focused on improving health equity and access for underserved populations.

The role includes 20% local travel, home visits and community outreach, with an hourly flat rate and mileage reimbursement. Candidates must reside in Northeast Ohio and be prepared to work 8:30 a.m. to 5:00 p.m., Monday through Friday.

Qualifications

• High school diploma or equivalent required.
• Ability to develop trust with staff, patients and families.
• Strong time management and communication skills.
• Bilingual in Spanish (preferred).

Responsabilités

• Learn core functions such as patient care, community outreach, and connecting patients with essential resources.
• Work closely with patients and community organizations to build trust and improve access to services.
• Develop relationships that enhance healthcare access and outcomes within the communities served.
• Gain understanding of cultural dynamics and partnerships between providers and community organizations.
• Assist in navigating the healthcare system to ensure patients receive needed support.
• Participate in outreach efforts to educate the community on health matters.
