Job summary

Oak Street Health in the United States is seeking a Community Health Worker to bridge patients, community, and medical systems, removing barriers and promoting health literacy. You will accompany patients through in-person and phone outreach, focusing on access to resources and care coordination, with flexibility to work outside the center.

Ideal candidates have at least 1 year in healthcare or community settings, strong communication, and the ability to travel.

Qualifications

• Minimum 1 year of experience in healthcare or community-based settings.
• Strong oral and written communication skills.
• Ability to manage multiple priorities with a positive attitude.
• Dedication to serving the community and building relationships.
• Proficient computer skills (Windows, GSuite, Microsoft, etc.).
• Access to reliable transportation and ability to travel throughout the community.
• US work authorization.
• Fluency in languages common in the community (e.g., bilingual English/Spanish).
• Experience on multidisciplinary teams with organizations and community members.

Responsibilities

• Establish and maintain strong relationships with patients and community partners.
• Manage patient referrals defined by the care team and coordinate action plans.
• Facilitate communication among patients, families, providers, and community orgs.
• Build inventory of local community organizations that may benefit patients.
• Connect patients to housing, transportation, food, and daily living resources.
• Assist patients with applications for eligible benefits and resources.
• Promote goal setting and achievement with the care team.
• Meet with patients in patient-centered locations and travel as needed.
• Document interactions in electronic medical records and maintain HIPAA compliance.
