Job summary

Oak Street Health is seeking a Community Health Worker in Philadelphia to bridge patients with healthcare and community resources. You will promote health literacy, assist with housing, transportation, and food access, and coordinate care with MSW and clinical teams.

The role requires traveling locally (about 50%), strong communication, empathy, and the ability to work within interdisciplinary teams. A CHW certification or related degree is preferred, and a $1,500 sign-on bonus is available.

Qualifications

• Minimum of 1 year of experience in a case management type position in healthcare, community-based or social services.

Responsibilities

• Connect patients with eligible state benefits and local community resources related to housing, transportation, food and daily living.
• Facilitate communication between patients, family, caregivers, medical providers and community organizations.
• Meet patients in preferred locations; 50% outside the clinic.
• Establish and maintain relationships with patients, community organizations and care team.
• Manage patient referrals and collaborate with MSW on action plans.
• Encourage health goals and celebrate achievements to improve outcomes.
• Engage high-risk individuals and accompany to appointments as needed.
• Participate in team meetings to support decision-making.
• Document interactions in EPIC and maintain HIPAA compliance.
• Work independently and with the care team to manage priorities.
