Salary Range: $19.00 Hourly

We are seeking a Community Habilitation Specialistto join our team of caring professionals.

FLEXIBLE HOURS!

The Community Habilitation Specialistreports directly to the Program Coordinator of Community Services. Responsible for providing support and assistance to the people we support.

RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO
  • Work one to one with the assigned participants (s) on the facilitative activities specified individualized service plan.
  • Travel into the community as required by the Community Habilitation Plan
  • Assist in the planning and/or review of the Community Habilitation outcomes along with family, participant(s), family advisor and program coordinator
  • Write comprehensive progress notes
  • Submit progress notes and time sheets on a weekly basis
  • Follow all specifies procedures especially those regarding emergencies and incident reporting. Reports incidents immediately
  • Communicate with the Director of Community Services regardingany concerns or problems with service and delivery
  • Mandatory annual training sessions.

Pay - $19.00 per hr.

Qualifications

  • High School Diploma or General Equivalency Diploma/Certificate
  • At least 1year experience working with individuals with Developmental Disabilities and or Physical Disabilities
  • Valid NYS Driver’s License and must have own vehicle.
