Qualifications

• Doctoral degree in Medicine with related Family Medicine experience.
• Board eligible or board certified in Family Medicine.
• DEA certificate is required.
• Ohio licensure or ability to obtain licensure.

Responsibilities

• Develop and implement individualized treatment plans.
