Qualifications

• 2 years of experience supporting people with developmental disabilities.
• Proven ability to work independently in home and community settings.

Responsibilities

• Provides person-centered ADL, health and safety support.
• Implements Person-Centered Plan goals and tracks progress.
• Documents services and reports health or safety concerns promptly.
• Administers medications within CMT scope and requirements.
• Supports community inclusion through recreation and social activities.
