Job summary

Triangle, Inc. is seeking a Community Coordinator to oversee participant groups across work sites and community outings, while developing engaging curricula and partnerships. You will transport participants, assist with assessments, and support independence and self-advocacy in daily activities.

Qualifications

• High school diploma or equivalent required.
• Some college coursework or 1–3 years of relatable experience preferred.
• Massachusetts driver's license and clean driving record required.

Responsibilities

• Supervises groups of participants in diverse settings including work sites and community outings.
• Develops and maintains diverse volunteer, vocational, and educational opportunities.
• Assists with development of new and engaging curriculum to meet program goals.
• Implements independent living, employment, self-advocacy, and soft skills into daily activities.
• Transports individuals to and from community activities and employment sites.
• Prepares or assists in required assessments and reports (incident reports, safety assessments, progress summaries).
• Participates in training and professional development opportunities.
• Intervenes in unsafe situations to ensure participants' safety.
• Participates in ISP meetings with DDS and families as needed.
• Collects accurate data for ISP, behavior plans, and progress notes per supervisor directions.
• Collaborates with other Community Coordinators and Program Coordinator to ensure quality service delivery.
• Reports progress and issues regularly to the Program Coordinator.
• Represents Triangle positively to customers, families, and community partners.
• Participates in biweekly team meetings and monthly supervision with Program Coordinator.
