Job summary

CareMore Health seeks a Community Outreach and Relations Consultant to advance value-based care through strategic community engagement. You will build partnerships with FQHCs, providers, advocacy groups, and public agencies to improve access, equity, and preventive care across markets.

You will analyze community health trends, translate insights into actionable plans, and collaborate with Marketing, Population Health, and Care Management to align engagement with priorities.

Qualifications

• Bachelor’s degree required; Master’s degree in Public Health, Healthcare Administration, Communications, Social Work, or related field preferred.
• 4–6 years of experience in community relations, outreach, public health, or related field.
• Advanced knowledge of community engagement strategies, health equity, and stakeholder management.
• Experience supporting value-based care, Medicare Advantage, Medicaid, or population health initiatives preferred.
• Strong ability to manage partnerships and influence across organizations.

Responsibilities

• Develop and execute community outreach and engagement strategies across markets.
• Build and sustain strategic relationships with FQHCs, community orgs, and providers.
• Represent CareMore Health at events to promote programs and preventive care.
• Analyze community health data and feedback to identify opportunities.
• Collaborate with Marketing, Population Health, Care Management, and Quality teams.
• Develop outreach plans, educational materials, and event strategies.
• Identify partnerships addressing barriers to care like transportation and housing.
• Provide strategic insights on community dynamics and health equity initiatives.
• Monitor outreach performance and referral activity; recommend improvements.
• Ensure activities comply with privacy, policies, and culturally competent engagement.
