Job summary

HopeTree Family Services is seeking a Community Engagement Professional to support adults with IDD in fully community-based settings. You will help individuals build independence, access meaningful activities, and develop skills for daily life.

The role covers Community Engagement and Community Coaching, focusing on person-centered planning, safety, and documentation. A valid driver's license is required to transport clients and represent the agency in the community.

Qualifications

• At least 1 year of experience working with individuals with IDD.
• Ability to transport clients and hold a valid driver's license.
• Strong organizational and communication skills.
• Ability to adhere to a service schedule and manage multiple priorities.

Responsibilities

• Facilitate participation in community activities and opportunities chosen by the individual.
• Foster natural relationships with community members beyond paid supports.
• Use community environments as learning settings to increase involvement and autonomy.
• Provide 1:1 coaching on skills like money management, safety, and transportation.
• Complete daily notes, incident reports, and required documentation on time.
