Job summary

Alliance is seeking an In Reach and Engagement Specialist for a hybrid role based in Johnston/Smithfield, NC. You will support members transitioning to community living, conduct assessments, educate on services, and collaborate with care teams across public and private partners.

Requirements include a high school diploma or GED and at least two years in behavioral health; preferred bachelor’s in human services. Hybrid/work-from-home options are available with occasional onsite meetings.

Qualifications

• High school diploma or GED required; 2+ years in behavioral health or related fields.
• Strong interpersonal and written/verbal communication skills.
• Experience coordinating care and connecting individuals to services preferred.

Responsibilities

• Complete Assessments and document results for housing and community services needs.
• Educate members about transitions to community living and available supports.
• Connect members to ongoing peer support and housing resources within Alliance.
• Collaborate with care teams and external stakeholders to expand community resources.
• Maintain caseloads, documentation, and ensure compliance with policies.
