In this highly collaborative role, you’ll work directly with patients and families to identify barriers that may affect their health—from transportation and access to follow-up care to food, housing and other social needs. You’ll connect patients with community resources, reinforce discharge plans and help ensure they have the support they need after leaving the hospital.

Work Hours: 8:00 am - 4:30pm

Hourly Pay Range: $25.52 - $38.28
This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
