Job summary

Fox Trail Memory Care Living in South River, NJ is hiring a Community Director – LPN to lead our team and support residents and families. This full-time role offers $85,000 annually and requires active NJ LPN licensure with leadership experience in healthcare settings.

You’ll supervise CMAs/CHHAs and activities staff, manage schedules, perform resident assessments, oversee medication documentation, ensure DOH compliance, and partner with families and marketing to maintain occupancy.

Qualifications

• Active LPN license in New Jersey.
• At least 1 year of experience in senior living, memory care, or home care.
• Administrative/management experience in a healthcare setting.
• Bilingual in Spanish preferred, but not required.

Responsibilities

• Supervise and support CMAs, CHHAs, and activities staff.
• Hire, onboard, train, and coach team members.
• Manage staff schedules to ensure adequate coverage.
• Complete resident assessments at admission, quarterly, and as needed.
• Oversee medication administration and clinical documentation.
• Ensure compliance with NJ DOH regulations and company policies.
• Communicate resident condition changes to families and providers.
• Oversee meal supply ordering and home cleanliness.
• Lead monthly special events and support daily activities.
• Serve as the primary point of contact for families.
• Conduct tours and partner with marketing to maintain occupancy.

What You'll Do:

What You'll Need:

• Active LPN license in the state of New Jersey

• Administrativeand/or management experience in ahealthcare setting
• Bilingual in Spanish preferred, but not required
• Strong understanding of dementia and Alzheimer's care
• Excellent organizational and communication skills
• Proficient in Microsoft Office and willing to learn internal systems
