Job summary

HELP Foundation Inc. in Lakewood, OH is seeking Direct Support Professionals to support day programs and community integration. The role includes driving 15-passenger vans, assisting with daily living activities, and implementing ISP-related tasks to help individuals participate in the community.

You will promote dignity, document services, and help participants grow health, wellness, and life skills. A HS diploma, valid Ohio driver's license, and weekday availability are required.

Qualifications

• High School Diploma or GED and 1 year of care experience preferred.
• Basic computer literacy and clear reading/writing skills.

Responsibilities

• Provide daily support and assist with activities of daily living as outlined by ISP.
• Transport participants using agency 15-passenger vans and ensure safety.
• Maintain accurate documentation and data collection for services provided.
• Support participants in community activities and enrichment opportunities.
