Job summary

Qualifications

• BA/BS degree or four years of equivalent education and/or experience in the human services field.
• Must pass KBI, DCF child & adult abuse checks, motor vehicle screen, and background checks.
• If a personal vehicle is used, reliable, insured transportation that is registered and compliant is required.

Responsibilities

• Provide face-to-face, community-based interventions as determined by the treatment plan.
• Help patients identify strategies or treatment options.
• Deliver solution-focused interventions and manage emotional/behavioral issues.
• Support patients to address functional deficits by setting goals based on strengths and needs.
• Assist patients in staying in community locations and develop crisis management plans.
