Job summary

Andwell Health Partners in Lewiston, ME is seeking a Care Manager Social Worker to coordinate patient-centered care for a defined population in collaboration with primary care practices. This role emphasizes housing support for HOME enrolled patients to reduce unnecessary hospital visits.

You will engage patients to improve overall health and well-being, work with MaineCare and providers to close care gaps, and support complex emotional-social needs in community and home settings.

Qualifications

• Bachelor’s degree in Social Work, Sociology, Public Health, or Nursing required.
• Combined five years of education and experience in providing direct services in social, health, or behavior health fields.
• MHRT/C Certification acceptable.

Responsibilities

• Coordinate services with primary care practices to provide patient-centered care.
• Engage patients to reduce unnecessary health care services and improve health; housing support for HOME enrolled patients.
• Assess gaps in care coordination with practices and community supports.
• Collaborate with physician practices and community partners to address needs of the most complex patients.
• Work with patients with complex emotional-social issues and mental health concerns.
• Identify recipients who would most benefit from care management and obtain consent to participate.
• Make patient visits prior to hospital discharge, in the primary care practice, in the home or public settings.

What You’ll Do

• Care Manager Social Worker will, in collaboration with the primary care practice, coordinate services using a systematic and cost-effective approach to provide patient-centered care for a defined population.
• The Care Manager will engage the patient to reduce unnecessary health care services and improve their overall health and well-being. A specialized focus on housing support will be provided to HOME enrolled patients.
• Care Manager to work with each practice to assess the gaps in care coordination as well as patients’ access to and utilization of community supports.
• The Care manager will need to collaborate with designated physician practices and community partners to determine the best model for addressing the needs of the most complex patients.
• Work with patients with complex emotional-social issues and mental health concerns.
• Work with CCT, primary care provider, and MaineCare to identify recipients who are most likely to benefit from care management intervention and agree to participate.
• Care manager may make patient visits prior to hospital discharge, in the primary care practice, in the home or public setting.

What You’ll Bring

• Must meet one of the following criteria:
• A minimum of a bachelor’s degree in social work, sociology, public health, or nursing from an accredited four (4) year institution of higher learning;
• Combined five (5) years of education and experience in providing direct services in social, health, or behavior health fields; or
• A current Mental Health Rehabilitation Technician/Community (MHRT/C) Certification.
• Must have a reliable automobile.
• Must have current Maine driver’s license.
• Evidence of automobile insurance.
• Annual flu vaccination required.
