Qualifications

• Bachelor's degree or equivalent in related field
• 5 years direct service in social/health/behavioral fields
• MHRT/C certification or equivalent preferred

Responsibilities

• Coordinate services with the primary care practice to provide patient-centered care
• Engage the patient to reduce unnecessary health care services and improve health
• Special focus on housing support for HOME enrolled patients
• Collaborate with designated physician practices and community partners
• Work with patients with complex emotional-social issues
