Acenda Integrated Health is seeking a RHC-SJ Community Care Partner in New Jersey. You will bridge community needs with program partners, mobilize outreach, and coordinate referrals while maintaining accurate activity records.

The RHC‑SJ Community Care Partner acts as a bridge between the community and other RHC‑SJ partners, facilitating outreach, mobilization, and referral services. The role involves supporting community‑led action, identifying resources, linking community members to support, and maintaining accurate activity records.
High school degree or equivalent combination of education and related experience. Demonstrated experience in community outreach, proficiency in Word, Excel, and Outlook, and ability to work independently and in a team environment.
Base Pay: $19.00 per hour
Schedule: 20 hours per week
