Job summary

Vitra Health in Massachusetts seeks 2 Community Support Specialists who are passionate about community engagement and helping people access care and resources. You will coordinate social services, work with nurses and case managers, and support program participants through assessments and care plans.

The role requires on-site visits in the field and in our Braintree HQ, strong communication skills, familiarity with medical terminology, and proficiency with Microsoft Office and Google Suite.

Qualifications

• One year of clinical experience with elderly or disabled populations.
• Demonstrated knowledge of medical terminology.
• Advanced proficiency in Microsoft Office Suite and Google Suite (G Suite).
• Valid driver's license with proof of insurance required.
• Bilingual in English and a secondary language preferred.
• High School Diploma Required.

Responsibilities

• Develop and maintain relationships with providers, healthcare organizations, and community service agencies to ensure comprehensive support for program participants.
• Coordinate delivery of social services and formulate individualized plans of care with nurses and providers.
• Maintain client charts and ensure documentation complies with policies and regulations.
• Provide culturally appropriate health education, information, support, and counseling to participants and caregivers.
• Travel to the Braintree HQ and field sites as required; perform on-site visits and community outreach.
