Job summary

Vitra Health in Boston, MA seeks a Senior Care Access Navigator to guide families through intake for long-term care services and connect them with available resources. The role emphasizes relationship-building with clients, caregivers, and community partners in a fast-paced environment.

Qualifications

• Bachelor's degree strongly preferred.
• 2+ years in sales, relationship management, or client enrollment.
• 2+ years' experience with Microsoft Suite.
• Second language strongly preferred.
• Excellent problem-solving, customer service, and communication skills required.

Responsibilities

• Direct client interactions and onboarding for long-term support services.
• Educate families on program policies and regulations.
• Maintain professional communication via phone, email, text, and mail.
• Identify and credential qualified caregivers for Family Care programs.
• Maintain confidentiality of protected health information.
• Travel to client homes as needed.
• Serve as brand ambassador and foster customer service culture.

The Opportunity

The Senior Care Access Navigator is a brand ambassador responsible for stewarding clients through the intake processforlong-term support services.This relationship-based role supportsVitraHealth's commitment to excellent healthcare delivery and growth goals. The members of this teamemploy astrategic approach to ensure families are onboarded quickly and compliantly, and the workflow reflects the daily needs of our consumers.

Key Responsibilities

• Strong ability to take initiative and confidently directs clientinteractions

• Maintain professional communication with prospective clients through various channels, including phone, email, text, and mailing, atappropriate intervalsand withconsistency

• Collect and confirms relevant consumer and caregiverinformation
• Support all steps of the intake process as needed, includingleveragingregional resources to support consumerexperience

What we are looking for:

• Second language stronglypreferred
• Bachelor's degree is stronglypreferred
• 2+ years prior experience withsales, relationship management, or client enrollmentpreferred
• 2+ years' experience and complete comfort using Microsoft Suite
• Excellentproblem-solving, customer service, and communicationskillsrequired
• Prior experience withHubspot, Salesforce, Optum,Wellsky,RingCentral orsimilar platformspreferred
• Demonstrated ability to work independently, and willingness towork in the fieldifneeded

Work Environment and Physical Requirements

• Office-based work in a clean well-lit environment with fluctuating temperatures near others.
• Client-facing field staff and community liaisons require frequent travel to client homes or community settings; ability to drive safely; work in client homes may involve varying temperatures, odors, allergens, pets, and other environmental factors.
• Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse.
• Requires lifting and carrying equipment and supplies weighing up to 35 pounds; requires pushing and pulling equipment and supplies weighing up to 35 pounds; requires walking and standing; requires frequent sitting more than 75% of the workday; requires the ability to negotiate stairs; requires visual acuity and manual dexterity to operate equipment.
