Job summary

Vinfen seeks a Health Outreach Worker in Massachusetts to collaborate with the Clinical Care Manager and CBFS teams to coordinate care for One Care enrollees with complex needs. You will maintain documentation, support ICP implementation, and facilitate referrals to improve outcomes.

Under CCM supervision, you will conduct home/community visits, educate clients on self-management, and assist with accessing services. Travel to Vinfen locations as needed is required.

Qualifications

• Minimum of two years of experience in Human Services.
• Experience with psychiatric disabilities and co-occurring disorders preferred.
• Care coordination experience preferred.

Responsibilities

• Engage clients enrolled in One Care and coordinate with the Clinical Care Manager.
• Maintain and update client documentation in Vinfen systems and the One Care Plan EMR.
• Facilitate implementation and monitoring of Individual Care Plans.
• Coordinate referrals with CCM, PCP and other providers to optimize outcomes.
• Conduct home and community visits as required under CCM supervision.
• Support clients in accessing health care services and accompany them to visits as needed.
