Job summary

CareSource seeks a Community Health Worker for Massachusetts (Suffolk / Norfolk / Surrounding Areas) to support enrollees with social determinants of health and access to care. You will conduct in-person or telehealth visits, help build holistic care plans, and document interactions in the CER with care team collaboration.

The role requires driving, meeting complex needs of members, and coordinating with PCPs and behavioral health resources. Flu vaccination and license requirements apply.

Qualifications

• High School diploma required; Associate degree preferred.
• Two years of outreach experience with clients who have complex needs.
• Four years in health plan or managed care settings preferred.

Responsibilities

• Review SDOH assessments and follow up based on enrollees’ needs and preferences.
• Conduct in-person, telehealth, or telephonic visits as needed.
• Document all contacts and interventions in the CER accurately and timely.
• Identify gaps in care related to SDOH and collaborate with the care team.
• Provide health education with the care team and PCP or specialists.
