Job summary

NextStep Care seeks an experienced case manager to coordinate care for members in the community, liaising with providers and families to ensure quality outcomes. You will educate clients on available resources and help develop and monitor care plans with a focus on accessibility and continuity of care.

You will collaborate with the care team, document interventions, and participate in after-hours coverage as needed. A bachelor’s degree in social work and related experience are required.

Qualifications

• Bachelor’s degree in social work or related human services field required with 2 years of work experience.
• Experience in social work, home and community-based services, healthcare or geriatrics preferred.

Responsibilities

• Acts as liaison between members and home and community-based service providers to facilitate quality and cost-effective care and outcomes.
• Provides education and support to members and families regarding community resources and services.
• Develops relationships with key stakeholders including primary care providers and HCBS.
• Makes regular contact with members via phone and home visits to monitor wellbeing and adjust care plans.
• Records and documents interventions and member information according to guidelines.
• Collaborates with the Central Intake and Assessment Team to ensure continuity of care.
• Identifies and resolves care plan variances to ensure effectiveness of care plans.
• Assesses member engagement and behavior to support health and functional status.
