Community Health Coordinator

JOB SUMMARY:

The Community Health Coordinator is responsible for recruiting, enrolling, and supporting pregnant and up to 12 months postpartum clients. The Community Health Worker provides care coordination, support, planning, and community referrals to program participants and their families.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Recruit and enroll women in need of prenatal care and support services via community canvassing and other outreach efforts.
  2. Assist in the identification of families at risk for infant morbidity, particularly where the child shows signs of failure-to-thrive, abuse, and neglect.
  3. Conduct home visits at client's residences for women enrolled in the program to provide health education and facilitate risk reduction activities.
  4. Develop realistic and attainable goal plans with family to address child development and social and emotional growth as well as parent/child bonding and positive interactions.
  5. Assist parents in developing formal and informal community supports.
  6. Make referrals to appropriate medical or social service agencies and resources according to participant needs and risk factors. Follow up on all referrals.
  7. Conduct assessments on clients and infants at appropriate intervals.
  8. Plan, implement, and facilitate parenting education activities using an evidence-based curriculum.
  9. Participate in community activities and events while encouraging participants to attend as well.
  10. Maintain a set caseload, which may fluctuate, based on funding.
  11. Act as an advocate for clients.
  12. Travel throughout service area required.
  13. Complete and maintain paperwork, reports, and records as required, meeting all deadlines to comply with audits of computer and paper files.
  14. Exemplify positive role modeling by professionalism, language, work ethic, and positive communication.
  15. Perform other duties as assigned.

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

Education: Minimum high school diploma. Has obtained a Community Health Worker certification through the State of Ohio Board of Nursing or will obtain the certification within one year of employment.

  1. Minimum of three years' experience in the housing, human services, healthcare, or other relevant field is required.
  2. Must possess excellent interpersonal communication skills and the ability to network with outside providers.
  3. Must be comfortable working in a diverse environment, value diversity, and promote equity and belonging.
  4. Has knowledge and understanding of the social determinants of health and how they relate to client care and recovery.
  5. Possesses a working knowledge of medical terminology.
  6. Demonstrates proficiency with Microsoft Word, Microsoft Outlook, and various database software.
  7. Must possess a valid and current driver's license.
  8. Experience: two (2) years of experience working in a position serving the community and/or teaching experience is preferred.
  9. Experience working in a government public health agency or educational agencies in Case Management and/or Social Work is preferred.
  10. Experience working with children in diverse populations is preferred. Bilingual is a plus.
  11. Must have good dexterity with the ability to lift 30 pounds, push, pull, carry, climb, balance, kneel, and the ability to sit, stand, walk, bend, and squat.
  12. Must be well organized, detail-oriented, and have the ability to maintain confidentiality.
  13. Must be able to work independently from general instruction.
  14. Good working knowledge of computers and Microsoft Office software preferred.
  15. To be eligible for hire, the selected candidate will need to pass background screening.
