Job summary

The University of Vermont Medical Center seeks an experienced health systems liaison to lead coordination at the intersection of Emergency Department utilization and post-hospital care. You will partner with ED teams, social services, and community partners to improve transitions and reduce avoidable visits.

As a Mobile Integrated Health team member, you will drive systems-based problem solving and help strengthen care pathways that support patients in the community setting.

Qualifications

• Bachelor's Degree required.
• Nationally Registered Paramedic Certification or nursing degree required.
• 5+ years' experience in the medical profession, including EMT/AEMT/Paramedic or RN.

Serves as a central patient care liaison across healthcare, social services, and public sector partners to improve care transitions, reduce avoidable ED use, and strengthen community-based care pathways. This position is a member of the Mobile Integrated Health team.
