Job summary

UVM Health seeks a proactive leader to coordinate care across the ED, post-hospitalization, and community-based services. You will drive partnerships, systems-based problem solving, and efforts to reduce avoidable ED use by improving care transitions.

You will serve as a central patient-care liaison across healthcare, social services, and public sector partners as part of the Mobile Integrated Health team in Burlington, Vermont.

Qualifications

• Bachelor’s degree is required.
• Nationally Registered Paramedic Certification or nursing degree is required.
• 5+ years’ experience in the medical profession, including EMT/AEMT/Paramedic or RN.
