Spring Health in New York is seeking a Community Care Advocate to connect employees to social determinants of health resources and workplace support. This onsite role will coordinate referrals for work-life, financial, legal, and basic needs while guiding access to mental health services and benefits.

The position requires a Bachelor’s degree in a related field with strong Spanish preferred; you will document outcomes, collaborate with internal teams, and promote the community care advocate

  • Assist in linking employees to mental health treatment options and guide through applications.
  • Document and track metrics/outcomes of referrals (reason, demographics, referrals, outcomes).
  • Represent Spring Health and support program parameters and goals.
  • Communicate needs to Spring Health partners through routine meetings.

Community Care Advocate

The Community Care Advocate serves aretail customer by connecting employees tocommunity and company resources to meet social determinants of health and needs of theemployee.

What you’ll be doing:

This is an onsite role: Monday 9a-9p, Tues-Thu 11:30-8, Friday 12-5 in zip code 12010

  • Identify and make referrals for work-life, financial & legal, and basic needs support.
  • Link employees to appropriate local and national resources via one-on-one consultationand referral list(s), which may include basic needs (housing, food, transportation, etc.),financial, legal, daily living and other worklife support resources and provide supportthroughout applications and follow-ups as needed.
  • Work with customer internal care teams on behalf of those employees in need ofemergency funding and resource referrals.
  • Offer consultation related to navigating mental health resources (local and national) andavailable workplace benefits, such as onsite Mental Health Coaches, work life articlesand guidebooks to the team members of the customer
  • Assist in actively linking employees to appropriate mental health treatment options (i.eSpring Health, behavioral health through medical insurance, emergent behavioral healthservices, and community resources) and provide support throughout applications andfollow-ups as needed.
  • Documenting and tracking of metrics/outcomes that include but not limited to; reason forsocial work involvement (resource need type), demographics of employees (asavailable), referrals placed (where, when) and success of referrals (connection rate,outcome of referral).
  • Represent Spring Health and help to support/reinforce program parameters and goals.
  • Communicate with local points of contact and cascade any needs to Spring Healthpartners through routine meetings and touchpoints.
  • Submit reporting documentation of all client and organizational activities on a routine andtimely basis.
  • Proactively develop relationships with key customer contacts and attend customermeetings as requested/appropriate.
  • Proactively promote the community care advocate service offering to employees.

What we expect from you:

A Bachelor’s degree in a related fieldProficiency in Spanish strongly preferred

  • Experience working with diverse populations and backgrounds within the social workindustry.
  • Experience and a depth of knowledge with connecting individuals to resources locallyand outside of your local communities.
  • Ability to balance the needs of multiple stakeholders.
  • Flexibility and creativity when approaching workplace challenges.

  • Experience helping organizations manage evolving workforce environments and normsis a plus.
  • Experience connecting individuals to resources quickly in times of crisis is a plus.
  • Experience assisting individuals through burnout, managing work-life balance, dealingwith trauma and/or traumatic events is a plus.
  • Experience working with lower income populations who experience challenges such asfood insecurity and housing instability is a plus.

The target salary range for this position is $28-$34/hour. Individual pay may vary from the target range and is determined by a number of factors including experience, location, internal pay equity, and other relevant business considerations. We review all employee pay and compensation programs annually at minimum to ensure competitive and fair pay.
