Job summary

The position emphasizes teamwork with health care professionals, confidentiality, and strong communication. Fluency in Spanish is a plus, and an associate degree in social services is required.

Qualifications

• Associate degree in social services; Bachelor's degree is a plus.
• Two or more years of experience in social services or related field.
• Knowledge of community resources relevant to social determinants of health.
• Fluency in Spanish is a plus.

Responsibilities

• Provide counseling and referral to help improve personal and social functioning.
• Advocate to obtain necessary services for patients.
• Collaborate with Community Health Educator to address unmet community needs.
• Coordinate referrals with community agencies.
• Document interactions and referrals per standards.
• Maintain patient confidentiality and professionalism.
• Track appointment compliance and document outcomes.
