Job summary

Woodhull Medical and Mental Health Center, part of NYC Health + Hospitals, seeks a Placement Support Specialist to establish supportive relationships with clients and advance their education and competitive employment goals in the community.

You will provide job placement support, coordinate with education programs, work with the mental health treatment team, and conduct employer outreach, spending most hours in community settings to help clients succeed.

Qualifications

• Bachelor’s Degree and two years of related experience.
• High School diploma with six years of related experience.
• Equivalent education/experience with at least one year on the bachelor’s path.

Responsibilities

• Establish supportive relationships with clients to achieve vocational and educational goals.
• Provide job development and job search services tailored to individual interests.
• Spend majority of work hours in community settings supporting clients.
• Conduct at least six employer contacts per week.
• Assist with education goals in the community with schools and colleges.
• Create and update individualized support plans with input from family and treatment team.
• Coordinate outreach and timely interventions to re-engage clients.
