Qualifications

• Bachelor’s degree in Social Work or comparable Human Services field is required.
• 2 years of direct service experience with youth and families preferred.
• Flexibility to work evenings and non-traditional hours.
• Valid driver’s license with reliable transportation and appropriate auto insurance.

Responsibilities

• Provide case management, transportation, and professional services to youth and families.
• Travel daily to deliver community-based services in compliance with contracts and regulations.
• Complete all required documentation and training promptly.
• Coordinate and monitor services across mental health, medical, educational, and social services.
• Maintain strong relationships with foster parents and families; support reunification and stable living arrangements.
