CHRIS 180's The R.I.S.E. Program seeks a skilled care coordinator to provide trauma-informed clinical services and community-based support to youth and families.

The role emphasizes hands-on engagement in homes and schools, with a strong focus on safety, collaboration, and cultural responsiveness. You will maintain a caseload of 20–25 families, lead Child and Family Team Meetings, develop a Plan of Care, and coordinate services across medical, educational, and social systems, ensuring timely

Qualifications

• Master’s degree required in Social Work, Counseling, or Marriage & Family Therapy.
• Fully licensed (LPC, LCSW, LMFT) preferred.
• 1–2 years’ experience working directly with youth and families.
• Valid driver’s license; dependable vehicle; travel required.

Responsibilities

• Provide care coordination and trauma-informed clinical services in community settings.
• Maintain a caseload of 20–25 families.
• Travel to homes and schools; at least 75% of services face-to-face.
• Lead Child and Family Team Meetings within 30 days of referral.
• Coordinate services across medical, educational, mental health, and social systems.
