Community-Based Mental Health Therapist
About the Position

We are seeking a dedicated and compassionate Community-Based Mental Health Therapist to join our mission-driven team. In this role, you will bring high-quality mental health care directly to the individuals and families who need it most—whether in their homes, schools, or other community settings.

Clinical Services
  • Conduct intake procedures, including all required forms and releases.
  • Provide diagnostic assessments to determine eligibility and treatment needs.
  • Facilitate individual, family, and group therapy using culturally responsive, evidence-based approaches.
  • Deliver crisis intervention, safety planning, and risk assessments when needed.
Treatment Planning & Collaboration
  • Develop collaborative treatment goals with clients and families.
  • Identify client strengths, needs, and barriers to inform the Individual Service Plan (ISP).
  • Modify treatment plans as needed and communicate progress during treatment team meetings.
  • Coordinate care with community partners, schools, and other service providers.
Documentation & Compliance
  • Complete treatment plans, progress notes, and discharge summaries in a timely manner.
  • Maintain productivity expectations and meet all state, federal, and funding requirements.
  • Participate in clinical supervision and ongoing professional development.
Community Engagement
  • Connect clients to community resources and ensure follow‑up on referrals.
  • Support clients navigating trauma, addiction, grief/loss, family conflict, chronic stress, and other challenges.
  • Provide services where clients feel most comfortable—home, community, school, or telehealth.
Qualifications

Bachelor's degree in Social Work, Psychology, Counseling, or a related mental health field.

  • Ohio licensure required: LSW, LPC, MFT (independent licensure preferred—LISW, LPCC, IMFT).
