Job summary

Community Behavioral Health Center in Northeast Ohio is seeking a licensed therapist for outpatient mental health care. Join a collaborative, community-focused team dedicated to high-quality client care and a strong support network.

You will conduct comprehensive assessments, develop individualized care plans, and document progress in our EMR system, contributing to improved client outcomes in a diverse adult population.

Qualifications

• Master’s degree in Social Work, Counseling, or a related field from an accredited university.
• Active LISW, LSW, LPCC or LPC license to practice in Ohio.

Responsibilities

• Collaborate with medical providers, case managers, and other professionals to deliver care for clients with acute and chronic mental health conditions.
• Conduct thorough initial and ongoing clinical assessments in addition to providing therapy/counseling.
• Develop and implement individualized care plans tailored to each client’s needs.

Key Responsibilities

• Collaborate closely with medical providers, case managers, and other professionals to deliver care for clients with a range of acute and chronic mental health conditions.
• Conduct thorough initial and ongoing clinical assessments in addition to providing therapy/counseling.
• Develop and implement individualized care plans tailored to each client’s unique needs.
• Maintain accurate and timely documentation of client progress and treatment activities in our Electronic Medical Records (EMR) system.

Qualifications

Education: Master’s degree in Social Work, Counseling, or a related field from an accredited university

Licensure: Active LISW, LSW, LPCC or LPC license to practice in the state of Ohio

Skills

• Strong assessment and diagnostic skills with demonstrable experience in evidence-based treatment modalities.
• Proficiency with electronic medical records and strong computer skills
• Effective communication skills and cultural competence in working with diverse social, racial, and ethnic groups.
• Preference for those with experience serving bilingual clients and/or using interpreters.
