Job summary

Cardinal McCloskey Community Services (CMCS) is seeking a Community-Based Service Provider to deliver direct services to children and families enrolled in CFTSS and HCBS. Using a person-centered planning approach, you will develop individualized treatment plans, engage youth and caregivers, and coordinate with schools and healthcare partners.

The role requires a Bachelor's degree, two years' experience, bilingual English-Spanish, and flexibility including evenings and weekends.

Qualifications

• Bachelor's Degree in Human Services or related field.
• Two years of experience working with children to support development.
• Knowledge of mental health diagnoses, trauma responses.
• Flexible schedule including evenings and weekends.
• Bilingual English-Spanish required.
• Strong communication and technology skills.

Responsibilities

• Provide direct, goal-focused services to assigned youth and caregivers.
• Develop and implement person-centered treatment plans based on strengths and needs.
• Assist youth and families in identifying and achieving treatment goals.
• Complete treatment plans, safety plans, progress notes, and documentation on time.
• Submit service-related notes within 48 to 72 hours of service delivery.
• Coordinate services and collaborate with schools, healthcare providers, community agencies, and other supports.
• Participate in interdisciplinary team meetings and case consultations.
• Maintain a service calendar documenting all scheduled, completed, missed, or rescheduled services.
• Communicate effectively with Managed Care Organizations and other stakeholders as required.
• Attend clinical supervision, service-based trainings, and all mandatory agency trainings.

As a Community-Based Service Provider (CBSP), you will deliver direct services to children and their caregivers enrolled in Children & Family Treatment Supports and Services (CFTSS) and Home & Community Based Services (HCBS) programs.
