Job summary

Clermont Senior Services Inc. is looking for a full-time Case Manager to serve the Senior population of Clermont County. The position requires candidates to work Monday through Friday from 8:00am to 4:30pm and possess a current licensure as a LSW, LPN, or RN.

Successful candidates will conduct initial assessments, prepare care plans, and arrange service delivery based on identified needs. Benefits include health insurance, retirement contributions, 11 holidays, and accrued vacation days.

• Knowledge of community resources and available funding sources.

Responsibilities

• Provide initial assessment of assigned customers to determine eligibility.
• Prepare a care plan in cooperation with the customer and caregivers.
• Arrange delivery of all services as identified on the care plan.
