Job summary

Ready 4 Work is seeking a Registered Nurse for the Kent County ACT Team in Delaware. The role is primarily Monday–Friday with an on-call component, serving adults in the office and community to advance treatment plan goals.

The ideal candidate holds an Associate's degree in Nursing, a valid Delaware RN license, and a valid driver’s license with reliable transportation. On-site with travel required to community settings in Dover, DE.

Qualifications

• Associate's degree in Nursing required.
• Must have a valid RN license in the State of Delaware.
• Valid Driver's License and reliable transportation.
• Must pass criminal background check and drug screen.
• Minimum 1 year of related nursing experience.

Responsibilities

• Visit clients to support treatment plan goals (e.g., nutrition, weight loss, medication education, exercise, benefits).
• Coordinate with other medical providers and accompany clients to medical appointments.
• Ensure medication orders are sent to the pharmacy and administered to clients.
• Take vitals monthly and administer IM injections.
• Work in office and in the community on a Monday–Friday schedule with on-call duties.
