Job summary

Apex Home Health, Hospice & Home Care seeks a Registered Nurse to provide community-based care for individuals with intellectual and developmental disabilities. You will conduct assessments, develop and implement care plans, monitor conditions, and collaborate with physicians to prioritize outcomes.

You will provide clinical oversight, medical assistance, staff training, and use critical thinking to meet patients’ biological, physical, and behavioral needs.

Qualifications

• Accredited RN degree and current state RN license.
• Minimum one year of clinical experience.
• Strong assessment, communication, teamwork, problem-solving and compassionate behaviors.
• Valid driver’s license and access to a registered vehicle with insurance.

Responsibilities

• Conduct assessments, develop and implement healthcare plans, monitor patient conditions, and collaborate with physicians and medical partners to prioritize patient outcomes.
• Provide clinical oversight, medical assistance, staff training, and use critical thinking to meet patients’ biological, physical, and behavioral needs.
