Job summary

Qualifications

• Requires High School Diploma or GED.
• Certified Peer Recovery Specialist (CPRS) preferred within 6 months of hire.

Responsibilities

• Acts as peer support throughout the patient's course of care and coordinates with the Care Transformation Organization.
• Documents and reports as required; conducts brief interventions for patients who screen positive for alcohol or drug use; identifies patient history and readiness to change.
• Participates in ongoing training to gain new skills.
• Provides family with recovery materials and conducts community-based outreach for linkage to services.
• Makes outbound calls to verify attendance with treatment providers and follows up with patients as needed.
