Job summary

Fernbrook Family Center seeks a Mental Health Practitioner to deliver skills-based services to clients in home and community settings under QMHP supervision. Duties include individual, family, and group modalities with a focus on daily functioning and resilience.

Flexible full- and part-time hours are available. Requires meeting productivity benchmarks and maintaining documentation; travel to client homes and Fernbrook locations is expected.

Qualifications

• Must meet one of Minnesota Statute qualifications for Mental Health Practitioner or Clinical Trainee.
• Reliable transportation and valid driver's license as required for community-based work.
• Criminal background check as per agency and DHS requirements.

Responsibilities

• Provide mental health services to children, adults, and families in home and community settings.
• Meet productivity and weekly billable targets.
• Use culturally responsive strategies in addressing client needs.
• Complete documentation per agency standards.
