Qualifications

• Must be at least 21 years of age.
• Bachelor's degree in behavioral sciences or related field from an accredited college or university.
• At least 2000 hours of supervised experience with persons with mental illness or DHS approved training on hire.
• Knowledge of available community services and connections.
• Flexibility and responsiveness to client needs.
• Criminal background check required.
• Valid driver's license and automobile insurance; reliable car for work.

Responsibilities

• Assess and review Diagnostic Assessment at start of services and create a functional assessment within 30 days, updating every 180 days.
• Develop ICSP within 30 days of first meeting and update every 180 days with goals and activity schedules.
• Perform referrals and linkage to maximize community integration.
• Monitor and coordinate services via telephone and other providers to avoid duplication.
• Develop crisis/safety plans and assist in managing client risk.
• Provide transportation as needed per ICSP.
