Job summary

Open Sky Community Services is seeking a Licensed Practical Nurse to join a patient-centered team in a non-hospital setting. You will collaborate with the Registered Nurse to meet medical needs and serve as a liaison with families and other providers.

The role emphasizes long-term patient relationships, nutritious meal planning, and adherence to health and wellness standards within a supportive, trauma-informed environment.

Qualifications

• High School Diploma, GED or equivalent.
• Certified Licensed Practical Nurse in Massachusetts, required.
• Excellent assessment, documentation, and verbal communication skills, required.
• Valid driver’s license and acceptable driving history, required.
• Experience with adults with disabilities, preferred.

Responsibilities

• Oversee medication ordering, counts and refills following physicians written orders.
• Monitor individual’s medical status and transport, or delegate transportation to and from medical appointments.
• Advocate for individuals with physicians and other service providers.
• In collaboration with the staff team, ensure health and wellness standards are being implemented including development of nutritious meals.
• Attend all ISP meetings and complete necessary documentation for the ISPs.
