Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts. The RN will provide monitoring, follow-up, and clinical care management for dual-eligible enrollees with complex needs, coordinating with primary care, specialists, and social services to improve health outcomes.

The role requires travel to member homes and community settings, strong communication, and the ability to lead an ICT to create holistic care plans and ensure seamless transitions across care settings.

Qualifications

• ASN degree in nursing required.
• RN license to practice in Massachusetts required.
• Prior care coordination/case management experience preferred.
• Medicare/Medicaid managed care experience preferred.
• Knowledge of dual-eligible populations preferred.

Responsibilities

• Engage enrollees in their homes and community settings to build care management relationships.
• Coordinate with healthcare providers and community resources to facilitate care transitions.
• Perform comprehensive assessments and functional assessments as required.
• Lead the interdisciplinary care team to develop holistic care plans.
• Oversee utilization of long-term services and supports to maintain preferred living arrangements.
