Job summary

Qualifications

• 1 year of experience in home health or related field.
• Graduation from an accredited nursing program; MO RN license.
• BLS certification required; valid driver’s license.

Responsibilities

• Provide day-to-day patient care in the home setting with high quality and safety.
• Perform case management, admissions, assessments, recertifications and discharges.
• Deliver patient education and ensure IV therapy, dressing changes, and site care per policy.
• Document care timely using the Epic system.
