Job summary

Communicare in Oxford, MS, is seeking a licensed or license-eligible behavioral health professional to provide evidence-based services in clients' homes or community settings. You will maintain caseloads, document care, and collaborate with the treatment team to coordinate services and ensure continuity of care.

The role requires a Master’s degree in a related field and licensure or eligibility for licensure within 30 days, strong communication, and ability to travel within the catchment area

Qualifications

• Master’s degree and license or licensure eligibility within 30 days.
• Ability to work independently and manage an active caseload.
• Strong interpersonal skills to engage clients and families.
• Willingness to travel across the catchment area for trainings and visits.

Responsibilities

• Provide evidence-based services in the individual’s residence or community settings.
• Arrange for medication evaluation, management and monitoring.
• Maintain an active caseload as directed by the supervisor.
• Meet all standards and policies regarding patient care and documentation.
• Provide timely services and documentation, and meet productivity requirements.
• Work in Communicare’s EHR system and participate in meetings/trainings as needed.
• Assist in data collection and contribute to the treatment team.
• Provide crisis services, intake, and therapy as directed by the supervisor.
• Assess home/family situations and involve families in treatment as appropriate.
• Coordinate with referral sources and other outside agencies on treatment plans.
• Collaborate with programs to ensure continuity of care and a coordinated system.

Education

Master’s Degree in counseling/related behavioral health field
