Mountain Home in Missoula, MT is seeking a Children’s Case Manager to deliver comprehensive case management to children 0–18, linking families to essential medical, educational, social, and recreational services. The CCM works mainly in the community and client homes, coordinating with therapists and providers, documenting thoroughly, and maintaining billing hours to support client progress.

Qualificações

• Bachelor’s degree in social work, psychology, rehabilitation counseling, education, vocational rehabilitation, or a related field.
• At least 2–3 years of experience in case management, mental health services, or related fields.
• Familiarity with Missoula community social services is preferred.
• IPS model familiarity is preferred.
• CPR/First Aid certification is preferred.
• State licensure or related credentials are a plus.

Responsabilidades

• Meet weekly with clients for case management and maintain client files.
• Build rapport with clients while maintaining professional boundaries.
• Provide services primarily in the community or clients’ homes.
• Coordinate referrals to mental health services, parenting, etc.
• Communicate with clients’ therapists for care coordination and treatment planning.
• Provide transportation and advocate for clients at appointments.
• Complete discharge processes and maintain billing hours.
• Attend meetings and training, and report to supervisor.
• Document dates, times, and progress notes in the electronic health record.
