Job summary

Assistance Plus is seeking a Children’s Case Manager (CCM) in Livermore Falls, ME to provide professional case management for children and adolescents. The role involves advocating for clients, coordinating services within an interdisciplinary team, and traveling within the designated area to meet client demand.

The position requires a minimum of a Bachelor’s in psychology or related field (or equivalent experience) and a Master’s in social work or related field in some cases, with a preference

Qualifications

• Minimum one year relevant life/work experience working with children with a Serious Emotional Disturbance is required.
• Must be 18 years of age or older.
• Intermediate level computer skills are required.

Responsibilities

• Provide professional case management service to eligible clients.
• Advocate for clients when securing services and coordinate all services deemed necessary.
• Coordinate services to meet the needs of clients served, working within an interdisciplinary team.
• Travel within the designated area and flexible hours to meet client demand.
